MAKATI Mayor Abby Binay today called on city residents to enroll or renew their membership with the PhilHealth ng Masa sponsored program at the Makati City Hall in order to avail themselves of subsidized medical care and other benefits it offers.
“I call on Makatizens to prioritize their registration with PhilHealth for their own sake as well as for the benefit of their loved ones. As an active partner of PhilHealth in making quality health care accessible to everyone, the city government aims to achieve optimum coverage especially among the less privileged residents,” Binay said.
New applicants and old members can enrol and renew their PhilHealth ng Masa membership at the PhilHealth Division located at the 4th floor of Makati City Hall Building I, from 8am to 5pm. The enrolment and renewal period started last January 3 and will end on February 28, 2017.
Makati Social Welfare Department (MSWD) officer-in-charge Ryan Barcelo said Makati residents may enrol and renew their PhilHealth ng Masa membership on scheduled dates, as follows: January 9 to 13, Barangays Poblacion, Valenzuela, Carmona, Kasilawan and Tejeros; January 16 to 20, Barangays Guadalupe Viejo, Pinagkaisahan, Pitogo, South Cembo and Northside.
January 23 to 27, Barangays Singkamas, La Paz, San Antonio, Palanan and San Isidro; January 30 to February 3, Barangays Comembo, Pembo, Southside, Cembo and Guadalupe Nuevo; February 6 to 10, Barangays Bangkal, Olympia, Sta. Cruz and Pio del Pilar; February 13 to 17, Barangays Rizal, West Rembo and East Rembo.
On February 20 to 28, enrolment and renewal will be open to new enrollees and members from the different barangays.
Barcelo said the requirements needed for renewal are Barangay Certicate of Indigency, Latest COMELEC Certificate, Certificate of Consultation from barangay Health Center, and Acknowledgment Receipt 2016 or PhilHealth ID 2016.
For 2017 PhilHealth ng Masa applicants, the following are the requirements: MDR (Member Data Record) or Request Form with Note: NO RECORD; Barangay Certificate of Indigency; and latest Comelec certification.
For married applicants, additional requirements include Marriage Certificate and Birth Certificate of dependent. For single with dependent, Birth Certificate of dependent is also required.
The benefits offered by PhilHealth to members and their dependents include Inpatient Care Benefits, Maternity Benefits including Newborn Care Package, and Cataract Package, among others.
As of December 2016, the city has over 22,000 Philhealth ng Masa Sponsored Program members.
