THE Insurance Commission (IC) reminded planholders of preneed company Danvil Plans Inc., which had been put under liquidation, to file their claims on their pre-need policies by July 30, 2015.
The requirements for the filing of claims against Danvil Plans are the policy face page consists of one page; the contract provision (consists of five pages); certificate of full payment (if the plan is already fully paid), and; photocopy of two government-issued identification documents with the photo and signature of the planholder.
In case of loss of any of the above, an affidavit of loss is needed. The IC said planholders should file their claims personally, or through a representative, or by mail/courier service at the following address: Danvil Plans Inc., second floor, PPSTA Building 1, Quezon Avenue corner Banawe Street, Quezon City.
For filing purposes, photocopies of the said documents may be used, but during the actual payment of the claims, the original copies of these documents must be submitted.
Those who have already filed their claims and were able to receive partial payment or installments do not need to file their claims again.
If a representative will claim the check, a special power of attorney is required.
Danvil Plans was put under liquidation as of December 16, 2014. As a consequence, its trust fund with trustee banks was put under the control of the IC and can only be withdrawn exclusively for purposes of paying benefits of its planholders.
